The Americans with Disabilities Act (ADA) protects people with disabilities from discrimination.
Disability rights are civil rights. From voting to parking, the ADA is a law that protects people with disabilities in many areas of public life.

FILING INSTRUCTIONS
In order to properly register a request, complaint, or grievance with the City of Baker, LA – ADA Coordinator, an individual shall obtain, complete, and submit the appropriate ADA form provided on this page.

If your report is related to Public Transit please utilize the CATS online form located at: CATS Title VI Complaint Form

By completing the applicable form and submitting online or utilizing other delivery methods, you can provide the details we need to understand what happened. You will receive a confirmation and a copy of your form when filed online. Your report is immediately sent to our staff for review.

Next Steps 

  1. Upon receipt of a properly completed grievance form, the ADA Coordinator will perform a preliminary investigation.
  2. If the complaint can be resolved to the complainant’s satisfaction during this stage, the resolution will be noted in writing and filed with the grievance.
  3. If the grievance remains unresolved, it will be submitted to the City of Baker Administration for resolution.
  4. The City of Baker Administration shall resolve to make recommendations on the grievance to the ADA Coordinator no later than 90 days after the complaint is received.
  5. The ADA Coordinator shall make a final decision on the grievance no later than 5 days after receipt of the recommendation from the City of Baker Administration.
  6. Notice of this decision will be transmitted to the complainant within 5 days after the decision has been finalized.
  7. A written record of the action taken on each request, complaint, or grievance will be maintained City of Baker, ADA Coordinator Office, and a resolution statement shall be sent to the complainant.
  8. Barrier removal requests will be evaluated by the City of Baker Public Works Department and ranked by the City of Baker Administration with regard to the resolution recommendation, budget, and scheduled projects.
